He absolutely is and shoulders the responsibility as well but I think this is fixable without firing the head coach.

Exactly. It looked to me like the guy was clearly in the end zone then grabbed the ball prior to reestablishing himself on the one.
Edit : I just rewatched it and it’s very clear that it was a touchback. A play like that can change the whole game

I think it might be the spot where the ball is that determines where the ball is placed.

^^ Followed up by Don Brown offense (3 yards and a TO) and a DB special teams ( penalty). DB was hired as Head Coach , not DC!

The coaching needs to teach these guys how to avoid meaningless penalties such as illegal formation.
